Membership Terms & Conditions Disclaimer

Membership benefits are subject to the following terms and conditions:

​

  • Unused monthly treatment credits may roll over for up to one additional month. Any unused credits will expire thereafter.

  • A minimum three-month membership commitment is required. After the initial three-month period, memberships may be canceled at any time with at least 30 days' advance notice.

  • Membership discounts do not apply to GLP-1 treatments, Hormone Replacement Therapy (HRT), peptides, laboratory services, or Wellness C infusions.

  • All treatments are subject to a medical screening and provider approval to determine eligibility and suitability.

  • Membership benefits are non-transferable, except where guest benefits are expressly included.

  • All treatments and appointments are subject to availability.

 

Wellness Life MedSpa reserves the right to modify membership benefits, pricing, and terms with appropriate advance notice, subject to applicable laws and existing membership agreements.
